Printable GLP-1 dose log

This guide explains how to keep a dose record. It is not medical advice. Dosing decisions belong to you and your prescriber.

A paper log is simple to hand across a desk at an appointment. This one has six columns, one row per injection, and prints on a single landscape page. The same fields are what Dial asks for when you log a dose on iPhone, so you can switch between paper and the app without changing what you write down.

What each column is for

Date and time

Write the moment you injected, not the moment you remembered to write it down. The U.S. labels measure their missed-dose and day-change windows in days or hours from the last injection (Ozempic and Mounjaro section 2.1, Zepbound section 2.3, Wegovy section 2.4). If you need those rules, read them in the label for your product or ask your prescriber. The log gives you the exact time to read them against.

Medication and strength

Copy the brand name and strength from the pen or carton, for example "Mounjaro 5 mg/0.5 mL". Pens of the same product come in several strengths, and a refill can change which one you have.

Dose as prescribed

Copy the dose from your prescription or pharmacy label. If the amount you injected differs from it, write the amount you injected and say why in Notes.

Injection site

Name the area and the side, such as "abdomen, lower left" or "right thigh". The injection-site rotation chart lists the areas each label names and has a numbered body map you can use as shorthand.

Notes

Use this for anything you will want to recognize later: a dose you skipped or took late, a new pen, a reaction at the site, or a question for your next appointment.

Keeping the same record in Dial

Dial's dose form has the same fields: medication, amount, date and time, injection site, and a note. For an injection, Dial asks for the site before it saves, and shows where and when you last injected. A dose you did not take can be logged as skipped, so the gap in your history has an explanation.

The free download keeps the phone log with no cap on entries. Dial Pro adds logging from Apple Watch, charts, and a PDF or CSV export of the same rows, which you can print or share if you choose.
